TERMS AND CONDITIONS

I, the applicant hereinafter called APPLICANT, hereby request to be supplied with gas and or water by the undersigned Dora Utilities, hereinafter called BOARD, for the purpose shown hereon and none other. For said gas/water I agree to pay at the regular schedule of rates published by Dora Utilities, of which rates are applicable from time to time fixed by the Board, and the Applicant further agrees to comply with the rules and regulations of the Board as prescribed thereby the Board. Applicant further agrees that (1) the Board shall retain title to all meters and other properties furnished by it; and (2) the Applicant shall be responsible for safekeeping of all property of the Board on the premises herein mentioned; and (3) that Applicant shall guarantee free right of ingress, egress and regress by the Board’s employees to meters, regulators, and other property of the Board located on said premises; and (4) Applicant will keep in repair all appliances and piping on said premises (other than meters and regulators maintained by the Board) first notifying the board prior to having repairs made, and will make written report immediately to the Board of any leaks discovered; and (5) that the Board shall not be liable for damages caused by the interruption of the supply of service by reason of fires, accident, repairs or from any accumulation of gas on the Applicant’s premises, and Applicant agrees to indemnity the Board against liability, loss or damage by reason thereof; and Applicant further agrees to notify the Board one week prior to vacating said premises or discontinuance of service for any reason; and (60 that the Board shall not refund any payment made by the Applicant or property owner for service pipe from property line to meter, unless covered by signed separate written agreement; and (7) that the Board shall have the right to discontinue service without further notice in case of Applicant’s failure to comply with this agreement or any part thereof or any part of the Boards General Rules and Policies. The Applicant further agrees that any unpaid bill may be deducted from any money paid to Board on the execution of the agreement.
